Welcome to Blue Sapphaire RCM LLC (“Blue Sapphaire RCM,” “we,” “us,” or “our”). These Terms & Conditions (“Terms”) govern your access to and use of www.bluesapphaire.com (the “Website”) and any services provided by Blue Sapphaire RCM LLC.

  • Our Services 

Blue Sapphaire RCM provides administrative and revenue cycle management support services, which may include:

Provider credentialing and re-credentialing
Medicare and Medicaid enrollment
Commercial insurance credentialing and payer enrollment
Medicare Advantage plan enrollment
Medical billing
Medical coding
Claims submission and follow-up
Accounts receivable management
Denial management and appeals support
Payment posting
Patient billing support
RCM consulting and reporting
Other administrative and healthcare revenue cycle support services

The specific services provided to a client will be determined by the applicable service agreement, proposal, or written arrangement between Blue Sapphaire RCM and the client.

  • No Guarantee of Payer Approval or Payment

Blue Sapphaire RCM will use reasonable professional efforts to assist with credentialing, enrollment, billing, coding, and revenue cycle activities.

However, we do not guarantee:

Approval by any insurance payer or government program
A specific credentialing or enrollment timeline
A specific reimbursement amount
Acceptance or payment of any claim
A specific increase in revenue
Approval of any appeal or reconsideration
Continued participation in any insurance network

Payer decisions, processing times, requirements, reimbursement policies, and claim determinations are controlled by the applicable payer or government program.

  • Client Responsibilities

Clients agree to provide accurate, complete, and timely information and documentation required to perform the requested services.

Clients are responsible for:

Providing accurate provider and practice information
Providing valid licenses, certifications, tax information, and other required documentation
Reviewing and approving information submitted on their behalf
Maintaining active professional licenses and required certifications
Providing timely access to necessary systems, portals, and information
Notifying Blue Sapphaire RCM of any changes affecting credentialing, billing, or enrollment

Blue Sapphaire RCM is not responsible for delays, errors, denials, or other issues resulting from inaccurate, incomplete, outdated, or delayed information provided by the client.

  • Fees and Payment

Fees for services will be communicated to the client through a written proposal, service agreement, invoice, or other written communication.

Current advertised pricing may include:

Medicare & Medicaid Enrollment: $200 per application
Commercial Insurance Credentialing: $150 per application
Medical Billing: 2% of collections, where applicable under the applicable agreement
Medical Coding: $5 per code, where applicable

Pricing may vary based on the scope of work, payer requirements, provider type, specialty, volume, complexity, and other factors.

All fees, payment terms, and billing arrangements are subject to the applicable written agreement between the parties.

  • Credentialing and Enrollment Services

Credentialing and enrollment applications may require information and documentation from the client. Processing times vary by payer and are outside Blue Sapphaire RCM’s control.

Blue Sapphaire RCM may assist with application preparation, submission, follow-up, and communication with applicable payers. However, payer approval is determined solely by the applicable payer or government agency.

  • Medical Billing and Coding

Medical billing and coding services are administrative support services. Clients remain responsible for:

The accuracy of clinical documentation
The medical necessity of services
Provider compliance with applicable laws and payer requirements
Final clinical and coding decisions, where applicable
Maintaining accurate patient and insurance information

Blue Sapphaire RCM does not provide medical advice, diagnosis, treatment, or clinical care.

  • HIPAA and Confidential Information

When our services involve protected health information (“PHI”), the parties may be required to enter into a separate Business Associate Agreement (“BAA”) or other appropriate written agreement, as applicable.

The handling of PHI and confidential information will be governed by applicable law and the relevant written agreements between the parties. Under HIPAA, a written business associate arrangement may be required when a service provider performs functions involving PHI on behalf of a covered entity.

Clients should not submit sensitive patient information through unsecured website forms, email, or other communication methods unless specifically instructed to do so through an approved secure process.

  • Website Use

You agree to use the Website only for lawful purposes.

You may not:

Use the Website for fraudulent or unlawful purposes
Attempt to gain unauthorized access to the Website or its systems
Interfere with the Website’s operation or security
Copy, reproduce, modify, or distribute Website content without permission
Submit false, misleading, or unauthorized information

We do not guarantee that:

Any particular business, financial, credentialing, enrollment, or reimbursement result will be achieved
12. Limitation of Liability

To the maximum extent permitted by applicable law, Blue Sapphaire RCM LLC and its owners, employees, contractors, and representatives shall not be liable for indirect, incidental, consequential, special, or punitive damages arising from the use of the Website or services.

Any liability arising from a specific client engagement shall be governed by the applicable written service agreement between Blue Sapphaire RCM and the client.

  • Indemnification

To the extent permitted by law, you agree to indemnify and hold harmless Blue Sapphaire RCM LLC, its owners, employees, contractors, and representatives from claims, losses, liabilities, damages, costs, and expenses arising from:

Your violation of these Terms
Your misuse of the Website
Inaccurate or incomplete information provided by you
Your violation of applicable laws, payer requirements, or third-party rights

We reserve the right to suspend or terminate access to the Website or discontinue services where permitted by applicable agreement or law, including in cases of:

Nonpayment
